Grand View Hospital’s DEM is all about patient care. The Nurses actively embrace any process that helps them improve the services they can provide. A big change was incorporating an ER Case Manager into the team. The nurses immediately began identifying patients with complicated discharge issues and alerting the ER Case Manager of families that needed additional support. The staff in the ER at Grand View are never too busy to recognize patients often have a multitude of needs that need to be addressed to maximize their recovery. Coordinating this change between the hospital’s Case Management Department and the Department of Emergency Medicine is a credit to both managers, Wendy Raifsnider and Jackie Howenstein. A successful integration would not have been possible without the enthusiasm of the ER staff – again, care in Grand View’s ER is all about the patient!
